Policy on Insurance

At Crawford Endodontics we make every effort to provide you with the finest care and convenient financial options. Staff are well informed and up-to-date  and can be reached by phone at 515-232-2090 with any questions or concerns regarding your initial visit. Please bring your insurance information with you to the consultation so that we can expedite reimbursement.

We are providers for Delta Dental, Wellmark Blue and UHC Insurance. Your reimbursement may not be equal to our fee. If you have another dental insurance, we work hand in hand with you to maximize your insurance reimbursement for covered procedures. Be assured we will do everything we can to make sure you are properly reimbursed by your insurance company.

If your dental benefit plan requires a "pre-determination" or "prior authorization", we will submit a treatment plan for review by your insurance carrier at no additional cost to you.

Again, we will help you in every way in filing your claims, handling insurance queries, processing follow-ups, lost claims, etc. Call any time you have a question. We are here to help you.
